AC2018Register and select the items you wish to receive. Please fill out and bring the attached Faculty Consent Form and a front/back copy of their insurance card.

If you ARE NOT covered with Aetna insurance, please fax (936-598-9208) or email a front/back copy of their insurance card with their DOB for verification of coverage to
csamford@auroraconcepts.net *If any insurance other than Aetna is not verified before the date of service we can not guarantee coverage and you may receive a bill. All insurance must be sent in no later than January 14 for verification. Labs are $65 if one DOES NOT HAVE insurance coverage.
